one thing that hasn't really been talked about is weight. is there a lot of dead weight in your car? (besides yourself lol).
i know when we had both strollers in the mazda, fuel consumption went way down. also, check tire pressure and make sure your engine bay is clean and free of debris. if you clog up your air intake with contaminates and the like, it will also affect your consumption.
the thing that strikes me is how it got worse over time. it was ok at the beginning but kept getting worse. i'm curious as to what the wear is like on your tires, as the balder the worse consumption. also, do you crank up the a/c?
oh, and never drive until the fuel light is on. it's not good for your injectors for multiple reasons. the light should only be on when you're already on your way to the gas station.
